  The goal of the SeniorLink is to link older adults within the Greater Kansas City community with a network of independent living services to assist them in maintaining and maximizing personal independence, increasing life satisfaction and decreasing health risks.  
 

We link people to the services they need to remain in the home of their choice.

SeniorLink, since 1993, has provided elder care consultation to seniors and their families to help them maintain the best quality of life possible.  We know that the segment of our population aged 60 or older is growing faster than any other.  Of those over age 85, more than 40% are estimated to be in need of some type of in-home assistance.  Simple tasks that one may have performed as a matter of course like housekeeping, grocery shopping, running errand or daily personal care may  become more difficult or impossible.

SeniorLink provides Service Coordination

With our network of more than 15 agencies in the Kansas City metropolitan areas, SeniorLink provides one central contact point for people  who wish to hire private home care services. SeniorLink care managers can assist you by: 

  • Providing assessment and consultation services to develop an initial plan of care.

  • Supporting your decision making by providing you with information about options for services, including agencies that can meet your care needs and their rates for service.

  • Coordinating the start of service and providing on-going support,problem solving and quality assurance follow-up.

The kinds of services which may be included in a plan of care are:

  • Housekeeping

  • Grocery Shopping

  • Personal Care

  • Sitter/Companion Services

  • Errands

  • Meal Preparation

  • Bath Visits

  • Adult Day Care

  • Live-in Care

  • Respite Care

SeniorLink provides Professional Care Management Services

At times, the support of a health care professional may be needed to provide more intensive follow up, to assist in resolving more complex care issues, or to provide medical support services.  SeniorLink’s staff of experienced medical professionals and social workers can assist with a variety of needs for persons living independently in their homes, and for those persons living in assisted living or nursing home settings.  Care management services can be helpful to you by providing:

Routine visits and monitoring for individuals whose family support is limited, or whose primary family support lives out of town.
  •  
Assistance with decision making regarding a move to or from a retirement community, assisted living, or nursing home
  •  
Routine medication setups.
  •  
Nursing consultation.
  •  
Physician/client liaison services
  •  
Consultation and assistance with crisis intervention
